    Nursing Services

    PRIMARY NURSE (CASE MANAGER)  / APPLY

    Position Summary:

    Functions as a member of an interdisciplinary team, provides case management and primary nursing care to assigned patients, addressing physical, psychosocial and spiritual aspects of care. Responsible for formulating and implementing the nursing plan of care that is consistent with standards of practice.

    Duties and Responsibilities:

    • Provides case management for hospice patients by coordinating with other disciplines to develop an interdisciplinary plan of care based on assessed needs of patients and family.
    • Assesses, plans, implements, and evaluates the nursing plan of care for assigned caseloads on a weekly basis.
    • Provides initial nursing visits within three days of admissions.
    • Continuously assesses patients for changing needs and adjust the plan of care accordingly.
    • Assists in the assessment of family need for bereavement services following the patient's death and convey information to the bereavement staff.
    • Identifies needs for additional services, including nutritional assessment, Physical Therapy, Occupational Therapy, and initiates appropriate referrals as needed.

    Minimum Qualifications:

    • Current state of Maryland Nursing license. CHPN preferred.
    • Graduation from an accredited Nursing program. B.S.N preferred
    • Minimum two years of acute care or previous experience in a hospice program.
    • Previous hospice or home experience strongly preferred.

    Casey House

    HOSPICE NURSE / APPLY 

    • Status: Part-Time/Non-Exempt
    • Benefits Eligible: Yes

    • Schedule: Friday and Saturday-7 p.m- 7a.m (works 24 hrs/ paid 30)

    Position Summary:

    Functions as a member of an interdisciplinary team, provides case management and primary nursing care to assigned patients, addressing physical, psycho-social and spiritual aspects of care. Responsible for formulating and implementing the nursing plan of care that is consistent with standards of practice.

    Duties and Responsibilities:

    • Provides skilled nursing care for all Casey House patients by performing a thorough assessment of patient's physical needs using effective nursing skills and judgment.
    • Ability to manage pain and control symptoms.
    • identifies learning needs of patients/ families by teaching proper drug scheduling, personal care skills, including cares of dying patients, the disease process and the effect and side effects of medications.
    • Accurately administers prescribed medications, both routine, and PRN base as indicated by patient assessment.
    • Performs specific treatments as ordered by the physician.

    Minimum Qualifications:

    • Graduation from an accredited nursing program, B.S.N preferred.
    • Current state of Maryland Nursing License. CHPN preferred.
    • Minimum one year of clinical nursing experience in medical-surgical, oncology, or critical care environment. Previous hospice experience strongly preferred. 
    • Possess excellent verbal and written communication skills.
    • Ability to communicate with patients and families under stress.
