Nursing Services

Primary Nurse, Hospice at Home / APPLY

Position Summary:

Functioning as a member of an interdisciplinary team, responsible for providing case management and primary nursing care to assigned patients and formulating and implementing nursing plan of care consistent with standards of practice.

Duties and Responsibilities:

  • Provides case management for hospice patients.
  • Assesses, plans, implements and evaluates nursing plans of care for assigned caseload on a weekly basis.
  • Provides primary nursing services for assigned patients.
  • Actively participates in and supports Shared Care model as adapted by Montgomery Hospice
  • Serves as contact person of the day, alternating with other team 
  • Completes all documentation within agency guidelines and in accordance with agency standards.
  • Maintains contact with primary physicians in accordance with agency policies and in keeping with physician’s request
  • Meets requirements for educational growth and development.

Minimum Qualifications:

  • Current licensure as an RN in Maryland, minimum of two years recent clinical experience.  Prefer critical care or previous experience in hospice or home care program. Requires strong critical thinking skills, EMR experience.
  • Significant understanding of practices, regulatory agency standards, and department/ organization policies and procedures related to the clinical program.

Clinical Nurse Manager, Hospice at Home/ APPLY

Position Summary:

The Clinical Nurse Manager will mentor and coach nursing staff.

Duties and Responsibilities:

  • Provide clinical expertise and promote best practice in the delivery of care. 
  • Performs joint visits with nursing staff. 
  • Assures that all documentation meets requirements for accuracy and timeliness.
  • Responsible for the day to day clinical activities of the hospice at home program and its staff.
  • Serves as a resource for evidence-based practice for the nursing staff.

Minimum Qualifications:

  • Current licensure as an RN in Maryland.
  • Minimum of four years of hospice experience, CHPN certified
  • Management experience preferred.
  • Demonstrates the ability to balance competing priorities and multiple projects concurrently. 
  • Participates in administrative on call coverage.

ADMISSIONS NURSE / APPLY

Position Summary:

The Admissions Nurse assesses patients and families who request admission to Montgomery Hospice. Provides a comprehensive assessment of patients to be admitted and initiates the plan of care consistent with the standards of practice, and functions in other capacities as needed.

Duties and Responsibilities:

  • Accurately assesses patients and family needs.
  • Develops an effective working relationship with representatives from other agencies who provide services for assigned patients.
  • Identifies and recommends medical equipment and supplies that will assist in maintaining a patient's comfort and caregivers' ability to provide care.
  • Identifies the need for additional services, including nutritional assessment, physical therapy, occupational therapy, and speech therapy and initiates appropriate referrals.
  • Assesses need for and educates caregivers on safety, and emergency preparedness, including safe use of medical equipment.
  • Conveys physician's requests to appropriate staff on appropriate forms. 

Minimum Qualifications:

  • Current state of Maryland Nursing License. CHPN preferred
  • Graduation from an accredited nursing program. B.S.N preferred
  • Two years of recent medical surgical experience, preferably with oncology/ multi-system failure
  • Previous hospice or home experience strongly preferred.

Weekend Visit Nurse, Hospice at Home / APPLY

Casey House

Position Summary:

Provides quality meals and nutritional services to patients and guests.  Maintains work area in a clean and healthy state according to sanitation regulations.  

Duties and Responsibilities:

  • Responsible for preparation of breakfast, lunch, dinner and snacks as planned by Consultant Dietitian.
  • Follows safe food handling standards.
  • May assist in the ordering of supplies and in coordinating cooks’ schedules​.

Minimum Qualifications:

  • Current Montgomery County certification as Food Service Manager.
  • Previous experience cooking in institutional setting required, healthcare experience preferred.

Administrative

Lead System Administrator / APPLY

Position Summary:

Contributes to the design, implementation, maintenance, troubleshooting, and upgrades of a primarily Dell, VMware, Microsoft, and Citrix based infrastructure in a critical role serving the organization’s 24/7/365 mission.   This position reports to the Director of IT in a department of four. 

Duties and Responsibilities:

  • Monitoring and testing of backup, disaster recovery, business continuity systems
  • Server, storage, network, security, telephony, endpoint health monitoring and reporting
  • Provisioning of new servers and application instances
  • Tier 3 support of the environment
  • Participation in weekly on call rotation (emergency-only after hours support – 1 week on, 3 weeks off)
  • Participation in six hour monthly off-peak maintenance window rotation (security patching, upgrades, DR testing – 1 day per month on, 1 month off altogether)
  • Continually develop knowledge of automation concepts and methodologies to standardize, streamline, and enhance IT processes through the use of Powershell and Cmd batch scripting.
  • Continually develop knowledge of cybersecurity and other best practices required of a HIPAA covered entity.

    • Powershell and Cmd scripting
    • Highly available WAN and private Ethernet switching and routing

Preferred Qualifications:

  • Bachelor’s Degree in Information Systems, Computer Science, or other related field
  • 5 years system administration experience in healthcare or other regulated environment
  • VMware, Microsoft, Citrix certifications
  • Experience with:
    • Dell Equallogic iSCSI SAN
    • VMware clustering, fault tolerance, high availability
    • Windows Server 2008 through 2016
    • Powershell scripting of Active Directory and Group Policy tasks
    • Exchange ActiveSync
    • Windows endpoint management tools (PDQ, SCCM)
