Nursing Services

Director of Nursing, Hospice at Home / APPLY

Position Summary:

The Director of Nursing will oversee the development of the Hospice at Home program including policies and procedures, coordinating nursing activities to ensure continuity of care.

Duties and Responsibilities:

  • Establishes, implements and evaluates goals and objectives for hospice at home services that meet and promote the standards of quality and contribute to the total organization.
  • Provides administrative oversight and assistance to the Clinical Nurse Managers in the development, organization, implementation, and evaluation of the Hospice at Home program.
  • Works collaboratively with the VP of Social Work and Counseling to oversee the interdisciplinary approach to IDG.
  • Provides guidance and counseling to managers to assist them in continually improving all aspects of hospice care services.
  •  Participates in administrative on call coverage.

Minimum Qualifications:

  • Current licensure as an RN in Maryland, with 5 years of hospice experience, CHPN certification, and management experience.
  • Significant understanding of practices, regulatory agency standards, and department/ organization policies and procedures related to the clinical program.

Clinical Nurse Manager, Hospice at Home/ APPLY

Position Summary:

The Clinical Nurse Manager will mentor and coach nursing staff.

Duties and Responsibilities:

  • Provide clinical expertise and promote best practice in the delivery of care. 
  • Performs joint visits with nursing staff. 
  • Assures that all documentation meets requirements for accuracy and timeliness.
  • Responsible for the day to day clinical activities of the hospice at home program and its staff.
  • Serves as a resource for evidence-based practice for the nursing staff.

Minimum Qualifications:

  • Current licensure as an RN in Maryland.
  • Minimum of four years of hospice experience, CHPN certified
  • Management experience preferred.
  • Demonstrates the ability to balance competing priorities and multiple projects concurrently. 
  • Participates in administrative on call coverage.

Hospice Nurse Instructor/ APPLY

Position Summary:

The Hospice Nurse Instructor will assist in the orientation and training of new nurses and the ongoing development of nursing staff.

Duties and Responsibilities:

  • Provide clinical expertise and performs joint visits with nursing staff to assess and improve the provision of care.
  • Assists Clinical Nurse Managers in Coaching new and existing staff.
  • Teaches case management skills to new nurses to develop their skills for mentoring new nurses.
  • Works closely with staff development.

Minimum Qualifications:

  • Current licensure as an RN in Maryland.
  • Minimum 3 years of hospice experience, CHPN certified.
  • BSN or higher preferred.
  • Experience in healthcare education preferred. 
  • Demonstrated knowledge of state, federal and regulatory standards, including The Joint Commission standards as applied to hospice programs.
  • Demonstrated excellence in written and oral communication.
  • Computer proficiency required.

ADMISSIONS NURSE / APPLY

Position Summary:

The Admissions Nurse assesses patients and families who request admission to Montgomery Hospice. Provides a comprehensive assessment of patients to be admitted and initiates the plan of care consistent with the standards of practice, and functions in other capacities as needed.

Duties and Responsibilities:

  • Accurately assesses patients and family needs.
  • Develops an effective working relationship with representatives from other agencies who provide services for assigned patients.
  • Identifies and recommends medical equipment and supplies that will assist in maintaining a patient's comfort and caregivers' ability to provide care.
  • Identifies the need for additional services, including nutritional assessment, physical therapy, occupational therapy, and speech therapy and initiates appropriate referrals.
  • Assesses need for and educates caregivers on safety, and emergency preparedness, including safe use of medical equipment.
  • Conveys physician's requests to appropriate staff on appropriate forms. 

Minimum Qualifications:

  • Current state of Maryland Nursing License. CHPN preferred
  • Graduation from an accredited nursing program. B.S.N preferred
  • Two years of recent medical surgical experience, preferably with oncology/ multi-system failure
  • Previous hospice or home experience strongly preferred.
