    Nursing Services

    PRIMARY NURSE (CASE MANAGER)  / APPLY

    Position Summary:

    Functions as a member of an interdisciplinary team, provides case management and primary nursing care to assigned patients, addressing physical, psychosocial and spiritual aspects of care. Responsible for formulating and implementing the nursing plan of care that is consistent with standards of practice.

    Duties and Responsibilities:

    • Provides case management for hospice patients by coordinating with other disciplines to develop an interdisciplinary plan of care based on assessed needs of patients and family.
    • Assesses, plans, implements, and evaluates the nursing plan of care for assigned caseloads on a weekly basis.
    • Provides initial nursing visits within three days of admissions.
    • Continuously assesses patients for changing needs and adjust the plan of care accordingly.
    • Assists in the assessment of family need for bereavement services following the patient's death and convey information to the bereavement staff.
    • Identifies needs for additional services, including nutritional assessment, Physical Therapy, Occupational Therapy, and initiates appropriate referrals as needed.

    Minimum Qualifications:

    • Current state of Maryland Nursing license. CHPN preferred.
    • Graduation from an accredited Nursing program. B.S.N preferred
    • Minimum two years of acute care or previous experience in a hospice program.
    • Previous hospice or home experience strongly preferred.

     

    VISIT NURSE  / APPLY

    • Status: Part-Time

    *Full Time- Monday- Friday (Will be periodically assigned to cover caseloads for absent Case Managers)

    *4:00 pm to 12 Midnight, Saturday and Sunday (Every or every-other-weekend)

    *12 Midnight to 8:00 am Saturday and Sunday (every-other-weekend)

    Position Summary:

    Provides patient care services. Makes planned and unplanned visits as assigned. Responsible for implementing a plan of care consistent with standards of practice. 

    Duties and Responsibilities:

    • Makes planned and unplanned visits to current hospice patients.
    • Ensures implementation of the plan of care.
    • Develops effective working relationships with representatives from other agencies who provide services for assigned patients.
    • Provides nursing services to hospice patients by effectively solving problems using critical thinking skills and able to demonstrate proficiency in anticipating symptom management needs. 
    • Provides initial assessment of hospice patients by coordinating with other disciplines as appropriate.
    • Completes all documentation within agency guidelines and in accordance with agency guidelines.

    Minimum Qualifications:

    • Possession of current advanced practice license with appropriate certification in the state of Maryland.
    • Certification in Hospice and Palliative Advance practice preferred.
    • Two years of recent medical-surgical experience, preferably with oncology/ multi-system failure.
    • Previous hospice or home experience strongly preferred.
