Nursing Services

Position Summary:

The VP of Nursing & Care Management is responsible for planning, coordination, direction, and evaluation of Hospice at Home and Casey House nursing services to ensure that the provision of patient care services meets established standards of practice, regulatory requirements and agency policies and procedures. As a member of the management team participates in agency-wide planning and operations. 
Duties and Responsibilities:
  • Plans, directs, and evaluates the delivery of nursing services to ensure that quality patient care is consistently delivered.
  • Oversees the management of the delivery of nursing services for Hospice at Home teams and Casey House.
  • Participates in the evaluation of organizational performance through quality improvement program and promptly addresses identified needs.
  • Ensures that a comprehensive infection control plan is in place.
  • Participates in establishing clinical policies and protocols which ensure that patient services are provided consistent with professional standards of practice and state and federal regulations and Joint Commision standards.
  • Monitors and evaluates contracted clinical services provided by outside agencies and professionals. 
  • Ensures that all nursing staff are aware of potential fraud issues and ensures compliance with all legal regulations.
  • Ensures that educational and professional development needs of staff are identified and are being met.

Minimum Qualifications:

  • Graduation from an accredited nursing program. BSN Preferred.
  • Masters degree in Nursing, Public Health, Health Administration or other relevant field is preferred.
  • Minimum of 5 years of progressively responsible hospice management experience.
  • Current nursing license from the Maryland Board of Nursing. CHPN required. Demonstrated clinical expertise, especially in physical assessment and pain assessment and symptom management.
  • Demonstrated knowledge of Joint Commision, state, federal, and regulatory standards as applied to hospice organizations.
  • Demonstrated leadership ability, and ability to clearly communicate ideas and instructions to staff.
  • Ability to effectively coordinate nursing services with other departments and services, inside and outside of the agency.
  • Demonstrated critical thinking skills and ability to exercise good judgment in emergency situations and ability to maintain effectiveness in a stressful environment.

Position Summary:

The Admissions Nurse assesses patients and families who request admission to Montgomery Hospice. Provides a comprehensive assessment of patients to be admitted and initiates the plan of care consistent with the standards of practice, and functions in other capacities as needed.

Duties and Responsibilities:

  • Accurately assesses patients and family needs.
  • Develops an effective working relationship with representatives from other agencies who provide services for assigned patients.
  • Identifies and recommends medical equipment and supplies that will assist in maintaining patient's comfort and caregivers' ability to provide care.
  • Identifies the need for additional services, including nutritional assessment, physical therapy, occupational therapy, and speech therapy and initiates appropriate referrals.
  • Assesses need for and educates caregivers on safety, and emergency preparedness, including safe use of medical equipment.
  • Conveys physician's requests to appropriate staff on appropriate forms. 

Minimum Qualifications:

  • Current state of Maryland Nursing License. CHPN preferred
  • Graduation from an accredited nursing program. B.S.N preferred
  • Two years of recent medical surgical experience, preferably with oncology/ multi-system failure
  • Previous hospice or home experience strongly preferred.

Position Summary:

The Professional Liaison will promote the value of hospice services offered by Montgomery Hospice by maintaining a professional presence in a hospital setting to facilitate the referral of patients to Montgomery Hospice from Physician practices, hospitals nursing homes, long-term care facilities, other community agencies and managed care companies. Provides information/ education related to hospice and palliative care to patients, families and hospital staff.

Duties and Responsibilities:

  • Promotes an increased understanding of MH services and programs to appropriate health-related audiences through one-on-one contact, attends meetings and responds to inquiries.
  • Meets with Physicians, hospital discharge planners, nursing home Social Workers, and liaison to assure that services are meeting needs of referred patients and families.
  • Attends discharge planning rounds, educational programs, case conferences and staff meetings at hospitals as appropriate.
  • Maintains records of all contacts and requests for information.
  • Participates in assessing, planning, implementation and evaluating policies to ensure efficient referral processes. 

Minimum Qualifications:

  • Graduation from an accredited Nursing program, B.S.N preferred or M.S.W with LCSW-C Maryland License.
  • Minimum 3 years of recent clinical experience required. Experience working in a hospice environment preferred.
  • Knowledge of state, federal and regulatory standards, including Joint Commission standards as it applies to hospice organizations.
  • Excellent written and communication skills strongly preferred.
